SPEAKER FORMS FOR CE SERIES WORKSHOPS

Thank you for agreeing to be a speaker/instructor for our NASW-AZ and ASU School of Social Work CE Series. Please complete the following forms so that we can prepare the online registration and put together a brochure. Thank you!

• Speaker/Instructor Agreement -- This must be completed immediately upon agreement to do the workshop.  This serves as the speaker contract. 
• Speaker Workshop Information Form  -- This needs to be submitted at the same time as the agreement so that we can market the program effectively. (If you are a new speaker for us, or if you have changed your tax I.D. number, this must be printed out, filled out, and faxed to us at 866-261-7588 or mailed to us at NASWAZ, 1050 E. Southern Ave, Ste. 2, Tempe, AZ 85282)
• W-9 Form -- (If you are a new speaker for us, or if you hae changed your tax I.D. number, this must be printed out, filled out, and faxed to us at 866-261-7588 or mailed to us at NASWAZ, 1050 E. Southern Ave, Ste. 2, Tempe, AZ 85282)
• Travel Reimbursement Form -- If you travel to present the workshop for us, then you may complete this Travel Reimbursement form after completing the workshop.
