Tani Bahti, RN, CT, CHPN has spent most of her nursing career focusing on improving end of life care through education. Recognizing how fear, misinformation and lack of information can negatively impact decision-making and the dying experience, she produced The Straight Talk Series on End of Life Issues and the award-winning video: Living Through Dying – The Struggle for Grace.  Tani has promoted clinical training programs for medical students and was founder of Passages – Support & Education in End of Life Issues, providing compassionate guidance in end of life issues through support, education & research.  She promotes community collaboration to creatively improve end of life education.


Learning Objectives

·         The role of the MSW in advocating for patient centered care

  • Maintaining your balance in dynamic and emotionally-charged family meetings
  • Enhancing communication between patient, family, and physicians
  • Identifying clear goals of care       

·         How quality of life perceptions transform over time - for those we serve    and ourselves.

  • Attendees will examine their own values and beliefs and impact on their work.
  • Recognizing the role of grief and loss and impact on caregiving
